Lutheran Homes Society, Inc. is currently searching for Occupational Therapists, Physical Therapists, Speech Language Pathologists, Certified Occupational Therapy Assistants and Physical Therapy Assistants to build our PRN/contingent pool. We have four long-term care locations (East Toledo, Holland, Sandusky, and Napoleon) where your skills can be utilized.

Coverage during the week is a current need, with weekend coverage needed as well. Successful candidates will be licensed in their appropriate discipline and have a strong commitment to quality care, with previous experience in a long-term care setting preferred.

 FYS - RT/DC Staff for YYA

Residential Treatment/Direct Care Staff for Youth & Young Adults

LHS Family & Youth Services requires strong, confident staff members, who have the capacity to deal with high-risk youth who have behavioral and emotional difficulties. Youth may experience frequent emotional crises and be assaultive and/or destructive. You must have the skills to verbally de-escalate a crisis and to use mature judgment with a youth who is out of control. You will need leadership skills, milieu management skills, high energy levels, self discipline, a cool approach to volatile situations, reliability, flexibility, and dedication.

We are currently hiring for 2nd shift and 3rd shift (must stay awake through the entire shift) positions. Experience working with high-risk youth, or a background in coaching, military or security is preferred. We are hiring for our facilities in Liberty Center, Ohio.  HCS - Maintenance Worker

Groundskeeper/Maintenance Positions

LHS Housing & Community Services has immediate openings for a full-time and part-time seasonal/temporary Groundskeeper/Maintenance person. Both positions require the individuals to work between various LHS housing locations in the Toledo, Ohio, area.  Some traveling will be required. The candidates will be responsible for the neat appearance of lawn, lawn edges, shrubs, trees, and other landscape areas. The candidates will also ensure parking areas are neat and clean and that driveways are free of debris, snow, and ice.

Another required duty will be preventative maintenance on lawn care equipment per manufacture specifications and with organized record keeping. The ideal candidates will have high regard for detail with focus on completing tasks timely and safely. We offer competitive pay in an environment dedicated to quality senior care.

Lutheran Homes Society is broadening its service line into the home health arena with the introduction of Northwest Ohio Home Care Partners. The expansion has created immediate needs for full-time, part-time, and per diem Occupational Therapists, Physical Therapists, Speech Language Pathologists, Certified Occupational Therapy Assistants, and Physical Therapy Assistants.

These positions will provide and direct therapy services to residents, including assessments, treatment, program planning and implementation, related documentation, and communication, primarily in a home care setting. Applicable licensure/certification for the appropriate discipline from an accredited program is required.  HCS - Transportation Driver

Transportation Driver

LHS Housing & Community Services has an immediate opening for a part-time Transportation Driver in Toledo, Ohio. Drivers are responsible for ensuring adequate and proper vehicle maintenance is conducted on a regular basis. Drivers will utilize ambulate vehicles to transport LHS housing residents on weekdays.

This position requires good oral and written communication skills. Drivers need to be 18 years of age or older and have had a valid driver’s license for at least two years. Drivers are not required to have a special driver’s license endorsement(s) or CDL to operate vehicles, but these are a plus. To be hired, drivers will need to pass a background check, drug screen, and obtain a signed statement from a licensed physician stating the driver has no medical or physical condition that may impair safe driving, passenger assistance, emergency treatment, or the health and welfare of a consumer or the general public. MDS Coordinator - RN

Lutheran Home at Toledo has an immediate opening for a part-time MDS Coordinator to work as a member of the nursing management team. This position is responsible for coordinating the development and completion of the resident assessment process in accordance with federal and state regulations. The successful candidate will have leadership skills, a current Ohio RN license, and strong assessment and communication skills.  Previous MDS experience and knowledge of MDS 3.0 is strongly preferred. MDS Coordinator - RN

Lutheran Home at Napoleon is looking for a full-time MDS Coordinator. Responsibilities will include: performing ongoing evaluations from pre-admission to discharge to ensure appropriate reimbursement levels for each resident; collaborating with other departments to ensure completion of resident assessments; leading the interdisciplinary team care conferences with residents and families; reviewing resident medical information to implement care plans and charting to support the care being provided; managing communication with managed care insurances and other payer sources; monitoring facility practices to ensure compliance with Medicare, Medicaid and other benefit programs; ensuring all new residents receive a timely MDS assessment; scheduling, completing and transmitting MDS reports in accordance with Medicare, Medicaid, OBRA and other program requirements; and anticipating additional assessments triggered by change in resident status.

Candidates must have a working knowledge of Medicare and managed care and a minimum of 1 to 3 years of MDS 3.0 experience.  This position requires a detail-oriented individual with strong organizational and communication skills. A RN is preferred, but a LPN with experience will be considered. We offer a competitive wage and excellent benefit package.
