Assistance for Toledo Area Seniors and their Families

Four part process:

  1. A “Needs Assessment” by a Licensed Social Worker (LSW)
  2. The Licensed Social Worker will make a recommendation about service options to the client.
  3. “LINC” client to appropriate affordable services enabling client to maintain independence
  4. Follow up case management if necessary and appropriate

“LINCing” Seniors to Area Services

There is no charge for the services of the LINCS coordinator; however, there may be a fee for the services obtained for the senior.

Referrals are accepted for any senior residing in the Toledo area. Area clergy, parish nurses, and community outreach groups are welcome to refer seniors to this program.

The LINCS ministry is also available to assist churches in developing programs to meet the needs of seniors in their congregation.

Services

We work with other agencies to “LINC” seniors to these services:

  • Assistance with transportation
  • Assistance with medical/insurance billing questions
  • Home delivered meals
  • Personal care
  • Housekeeping
  • Accessing prescription assistance plans
  • Finding affordable senior housing or long term care
  • Legal services
  • Professional Staff
  • Experienced and Licensed Social Workers
  • Certified OSHIIP Counselors trained to assist with Medicare/Medicaid and health insurance questions
