A: Extended care is Lutheran Homes Society’s most intensive level of care. Fully staffed, 24 hours a day with registered nurses, licensed practical nurses, and nurse aides, extended care is designed to meet the needs of the individual resident.
Q: How do I know if my loved one needs extended care?
A: Your loved one will meet one or more of the following criteria:
Individual can no longer care for him/herself
Individual needs therapy
Individual needs continuous medical supervision

Assessments by nursing staff

The Napoleon Center offers Medicare skilled care in a homelike environment. The staff of highly skilled healthcare professionals is dedicated to the residents. The tremendous volunteer base, provided through the auxiliary, is one of the outstanding attributes of the Home. Residents benefit from volunteers that assist with activities and take a genuine interest in their well-being. A wide range of activities is provided for all residents to help keep them as active and involved as possible.

Initially called the "Lutheran Old Folks' Home," the Toledo campus has undergone many changes through the years and has emerged as a highly-respected facility in the area. What makes the Toledo Home so unique is its full range of activities. Residents participate in activities such as ceramics, crafts, and quilting. There's even a special cookie bake room where residents share their culinary expertise to create desserts that rival the oldest and best bakeries in the country.

Located in Sandusky, Ohio, Lutheran Memorial Home features restorative therapy that helps residents develop the energy to enjoy activities. The program strengthens the heart and lungs, reduces stress levels and helps control weight. These are all important care components that allow residents to maintain their highest practical physical status. Along with the care and services provided to meet physical needs, there are also opportunities to promote spiritual growth, participate in activities, outings and family functions.

Located in Holland, Ohio, Wolf Creek offers the distinct advantage of total health care in partnership with one of the area's leading health care providers - St. Luke's Hospital. The complete medical services of St. Luke's Hospital are available to all Lutheran Village residents - on-site as well as on an outpatient basis at St. Luke's. Residents also enjoy nutritious, homemade meals prepared by our executive chef and served in one of two beautiful spacious dining rooms overlooking a landscaped terrace and wooded ravine.
